**THE ROLE**:
**£28,080 OTE (£24,000 - £26,000 Basic Salary + Bonus)**

Unity Care Solutions are an award-winning care company with 3 branches across Southampton, Kent and Sussex. We specialise in supporting children and adults in the community with complex care needs.

We are on the lookout for a **Care Coordinator** to join our experienced office team based at Ocean Village, Southampton. Having a passion for care and experience within a similar role is a must

**THE IMPORTANT STUFF**

Working hours are **Monday-Friday 9am-5pm**. We offer a competitive salary package of **£24,000-£26,000 per annum** with the opportunity to earn a **quarterly KPI bonus**

**THE DAY JOB**
- Liaise with carers, nurses, service users, social services, and other professionals to deal with any queries or complaints.
- Organise weekly rotas, considering any annual leave or sickness.
- Support our nurse case managers with the initial care plan assessments and care plan reviews.
- Conduct interviews, inductions and communicate with recruitment with new starters.
- Calculate staff gross payroll and reports in line with our company timescale to meet payroll on a weekly basis.
- Participating in the on-call rota out of hours.
- Ensure you are completing all tasks in line with the companies policies and procedures and any other relevant regulations and guidelines.
- Carry out spot checks and supervisions.

**WE'D LIKE TO SPEAK TO YOU IF YOU HAVE...**
- Experience as a care coordinator or within a similar role in the care industry.
- Good literacy and numeracy skills and are computer literate.
- Understanding of CQC and legislation concerned with care provision.
- A full driving Licence.
- NVQ Level 2 in Health and Social Care (NVQ Level 3 desirable)
