The Reconnect team aims to support individuals with identified needs on release from prison or returning to Hampshire from an out of area prison location. Reconnect will operate under the wider auspices of the Hampshire and IOW Liaison & Diversion Service.

Do you believe that a person’s health needs, and past experiences can be linked to their criminal offending? By identifying and reducing health needs can vulnerable people be supported to make positive changes and reduce the risk of reoffending?

You will work with Peer Support Workers who have lived experience to engage with those service users who are most difficult to engage to identify and support their unmet needs as they leave prison. With good leadership skills and compassion, you will provide a supportive network for peer
support workers to develop and grow a peer led approach to bridge the known health inequalities of prison users.

To support with the ongoing quality assurance of the Reconnect within the Liaison and Diversion Service in Hampshire.

To supervise Peer Support Workers in their role of using their own lived experience to support those with health vulnerabilities who are currently in contact with criminal justice
services.

To proactively make preparations with persons with assessed health vulnerabilities, as identified by clinicians, that are due to leave HMP Winchester to assertively support their engagement with services and support networks in their local community.

To use a person-centred approach to promote the continuity of established, positive working relationships during pre-sentencing phase of the criminal justice system. The post-holder will strive to maintain and build upon this existing relationship post sentencing. This will involve allocating cases and tasks where appropriate.

To form and utilise effective working relationships with partner agencies, such as the
police, court staff, probation services, mental health services etc

To be adaptable in the role to be able to offer support to meet the wider liaison and
diversion service needs, when required.

Collect and utilise service user experience feedback.
