We are seeking a compassionate and dedicated Outreach Worker to join our client's dynamic team based in Kingston. In this pivotal role, you will have the opportunity to make a significant impact by supporting the rough sleeper community and providing essential guidance on available options, safety measures, and listening to their concerns. Additionally, you will play a crucial role in maintaining and updating the CHAIN database, a vital tool that supports the outstanding work carried out by this charitable organization in the borough of Kingston.

As an Outreach Worker, you will work collaboratively with other compassionate individuals and agencies, united in the common goal of reducing homelessness in Kingston., + Assist rough sleepers in accessing appropriate social care services and exploring various accommodation options.
+ Act as a first point of contact for referrals, conducting needs and risk assessments for new rough sleepers.
+ Update assessments for rough sleepers already known to the team.
+ Regularly update and maintain the CHAIN database.
+ Determine the most effective course of action for each client's unique circumstances.
+ Collaborate with local authorities, health services, housing providers, the police, and drug & alcohol services to coordinate support for rough sleepers.

+ Exceptional interpersonal skills, allowing you to build rapport and trust with rough sleepers.
+ Flexibility and responsiveness to meet the diverse needs of individuals experiencing homelessness.
+ Confidence and capability in managing a caseload of complex clients.
+ Proficiency in IT skills to accurately collect, organize, and interpret data.
+ Strong representation skills to engage effectively with external meetings and agencies.
