Band 4 Rehabilitation assistant, Discharge to Assess, Central London Community Healthcare NHS Trust (CLCH) - Wandsworth. Fulltime

Are you a Band 4 or an experienced Band 3 rehabilitation assistant looking for an exciting, new challenge and a chance to take on greater responsibilities? We are looking for a highly specialist, dynamic and motivated rehabilitation assistant with excellent skills and a positive attitude and commitment to providing excellent holistic community-based care. The person will join our group of AHP staff in a permanent role and supporting an MDT community therapy service.

Discharge to assess (D2A) is a 7 day a week community assessment and rehabilitation team in Wandsworth. It provides therapy services to specific types of patients upon discharge from hospital. These patients are often more urgent and/or complex upon discharge to the community. The team supports the discharge to ensure readmission is avoided. This involves risk management, disability management and rehabilitation. The team sees a wide variety of conditions/pathologies and provides therapy in a holistic way using a wide variety of knowledge and skills. The team consists of Physiotherapists, physiotherapists and rehabilitation assistants in a multidisciplinary team.

You will be expected to work as an integral part of a D2A team.

Undertaking 1:1 hands-on work with patients to help them engage and practise specific interventions and

Techniques as outlined in the treatment plan as prescribed by therapist (Occupational/Physiotherapist).

reporting back to the therapists on their progress and any issues arising.

Completing environmental risk assessment together with the therapist in the patients home environment.

Supporting patients to adhere to therapy recommendations.

Engaging in daily rehabilitation as prescribed by Occupational and physiotherapist.

Engaging patients to undertake their exercise programme as prescribed by the therapist.

Preparing for and assisting with the moving and handling of clients during Therapist-led treatment

sessions.

Documenting progress of patients and giving feedback to therapist highlighting issues of concern to patients.

Recognising when clients are experiencing barriers to communication, and with guidance from

therapists utilise appropriate aids and techniques.

Writing clear, concise, accurate and timely client electronic records.

Work closely with therapist to implement treatment planning and implementation of agreed goal orientated intervention with Patients.

- To contribute to the on-going assessment of a patient by completing the appropriate paperwork for allocated patients, recording and reporting all clinical and behavioural information to the nurse/therapist for action and planning purposes.
- To undertake, and report back on delegated clinical activities that have previously been signed competent. E.g.: vital signs, gait/mobility training, falls prevention groups, increasing independence through activities of daily living
- With the appropriate training the RA will plan, design and deliver higher level strength and balance groups for patients/clients with a history or high risk of falls who have been identified as suitable for group exercise by the Falls Specialist Therapists.
- Within the remit of the higher level strength and balance group, review clients’/patients’ exercise programmes.
