**Place of Work**: Islington Central 1 PCN

**Hours**:Full time (37.5 hours a week) - Part time and job-share will also be considered and supported

**Salary**:£26,000 - £28,000 per annum (depending on experience)

**Duration**:Permanent

**About Islington GP Federation**
Islington GP Federation (IGPF) is a new, rapidly growing organisation representing 31 practices; we have established ourselves as a leader in new ways of working, including running Islington’s extended access primary care services (I: HUB) as well as supporting the Islington Primary Care

Networks (PCNs). Our current range of services includes I: HUB, community ENT, community gynaecology, practice-based pharmacists, hostel outreach program, long term condition management optimisation, proactive screening and evaluation trials, and a range of general practice support propositions. IGPF is host to Islington’s Training Hub.

IGPF works very closely with a range of partners including the regional commissioning group, NHS
England, Healthy London Partnership, Public Health, local hospitals such as the Whittington Health and the London Borough of Islington.

The PCN workforce is hosted by IGPF and over the last few years IGPF has been working with the
Islington Training Hub, to create training and development programmes that meet the need of all staff working in primary care.

**About Islington PCNs**

Central 1 PCN consists of 7 practices in the Central Locality of Islington and is supported by the
Islington GP Federation. The PCN group together independent GP practices, brought together by the
Network Contract DES. Central 1 PCN has a focus on improving patient care by working with partner providers to manage the needs of their local patient population.

The population within Central 1 PCN geography is diverse, often with significant and complex health and wellbeing needs. There is an exciting opportunity for enthusiastic and motivated individuals to support the co-ordination of this care and working closely with people to support them to achieve

Job Purpose
- Enable equitable access and uptake of effective health care for all
- Reduce health inequalities
- Improve patient and carer experience of primary care services and help personalise care
- Support the primary care value of continuity of care and relationship
- Contribute to and support the integration of health and social care aspects of patient care
- Achieving PCN DES targets
- Integrating with and support GPs and Practice teams

**Job role**
- They will work closely with GPs and practice teams, ensuring appropriate support is available to people; supporting them to understand and manage their condition and ensuring their changing needs are addressed.
- They will enable people to access the services and support they require to meet their health and wellbeing needs, helping to improve people’s quality of life.
- They will work alongside social prescribing link workers and health and wellbeing coaches to provide an all-encompassing approach to personalised care.
- The postholder will work with a diverse range of people from different cultural and social backgrounds. The ability to work confidently and effectively in a varied, and sometimes challenging environment is essential.
- This role will offer flexible and proactive support to vulnerable patients. Vulnerability can be regarded as any factor which increases a person’s risk of not being able to negotiate current care and service pathways safely, independently or advocate effectively on their own behalf. Examples are o Frailty and elderly o SMI and Personality Disorders o Dementia o Lives alone o LD o low levels of literacy or requires a translator o Support multi morbidity reviews o Digitally excluded o In-house point of contact for complex cases o Help with social services reports o English Second Language
