**Job Title**: Care Co-Ordinator

**Location**:Trinity Independent Care - Exeter Branch - Community and Office based

**Responsible to**: Care Manager and Registered Manager

**Salary**: £16.00 per hour of contact time.

**Hours**:37.5
- You will work alongside the Seniors to coordinate and complete initial assessments and risk assessments.
- You will work alongside the Seniors to coordinate, develop, and oversee the creating and reviewing of all customer plans and assessments.
- You will be responsible for creating and updating of the roster.
- You will be required daily to network and communicate with staff, customers, family, and wider professionals.
- You will need to complete assigned audits on schedule.
- You will cover sickness, absence and make required changes to the roster when needed.
- You will work to search the unsourced list and take on new packages to grow the company.
- You will need to participate in the on-call rota and all responsibilities associated with this.
- You will be required to support the learning and development of the staff alongside the Training, Development and Wellbeing Manager.
- You will be required to complete staff supervisions.
- You will be responsible for the general running and updating of the online care system. This includes inputting, changing, and analysing E-MAR’s, customer information, staff information and uploading relevant documents.
- You will be responsible for updating and overseeing quality improvement tools such as the Spot-check, Supervision and Customer Matrix.
- You will always work in line with the company policies and procedures.
- You will keep an accurate record of communications with staff, customers, customers family and other wider professionals.
- You will be required to problem solve and deal with informal complaints when needed.
- You will liase and work with other managers and staff within the company to ensure the quality of the service.
- You will cover Care calls as and when required

specified registered purposes and to use or disclose data only to authorized persons or

organizations as instructed.
