A new opportunity has arisen for a Care Coordinator who will share our core values, ethos and passion for delivering high-quality care at our Camden branch.

**Required Availability: Monday
- Friday (9:00- 17:30)**

**Care Coordinator Role & Responsibilities**:

- Previous experience in care coordinating preferred but not essential.
- Planning and coordinating schedules that motivate care workers and encourage service users’ needs and requirements.
- Ensuring that all care staff deliver high-quality and personalised care to all service users.
- Liaising with local Healthcare bodies such as, Hospital, Social workers and Brokerage officer’s organisations and maintaining good relationships with all.
- Confirming that all activities such as personal care and medication administration meet the required standards
- Preparing reports when and if required for all service users and care staffs
- Utilising resources and staying updated with the clinical knowledge to ensure that patients receive the best care.
- Ability to report all urgent issues to duty social workers.
- Knowledgeable or familiar with medical terminology
- Experience using access planner would be an added benefit but not essential.
