Working to the practice protocol ensure sets of medical notes are sorted in date order or prioritised appropriately. Enter all relevant clinical data on to the computer records accurately using the correct priorities. Remove any entries that are not in line with the practice protocol. Ensure that any patients with long-term conditions, immunisations or cervical cytology are entered in the recall system by liaising with the admin team.
Bring to the attention of the line manager and/or clinical lead any concerns/issues from medical notes that may need to be urgently addressed. Replace degraded events and allergies with an appropriate code. Prevent/allow viewing of a document/Consultation when either summarising/Coding/ Reviewing online access when necessary. Identify relevant clinical information contained in patient correspondence.
Code or free text the clinical information on to the patient record. Deal with queries relating to coding. Send on the clinical information to the relevant clinician as appropriate. Generate blood forms/ CXR requests.
Send on task to contact patient or text patient directly.
