Bush and Company Rehabilitation is seeking a skilled Clinical Associate Case Manager Specialist to support individuals with complex needs. This role involves coordinating care plans, managing individual care goals, and working closely with healthcare professionals.

About the Role

The Clinical Associate Case Manager Specialist will be responsible for assessing and managing individual care plans, coordinating with healthcare professionals to ensure seamless care delivery, developing and maintaining relationships with service users, families, and carers, and monitoring and evaluating care plans to ensure effective outcomes.

Requirements

To be successful in this role, you will need to possess a Registered Physiotherapist, Social Worker, Occupational Therapist, or Registered General Nurse qualification, have a minimum of 1 year of experience in a clinical setting, demonstrate strong communication and interpersonal skills, and be able to work independently and as part of a team.

What We Offer

We offer a competitive salary of $80,000 - $110,000 per annum, depending on experience, and a supportive work environment that encourages professional growth and development.
