**To deliver these post holders will**: Support the development of the Live Well model of care and the Alliance Delivery Plan. Develop partnerships between West Suffolk facing organisations, district and borough council teams and the voluntary, community, faith and social enterprise sector. Lead a range of service development and redesign programmes from concept through to delivery, against the main objectives in the Alliance Delivery Plan and Joint Forward Plan. Plan and organise a programme of schemes that deliver efficient and effective pathway and system changes Exercise a high degree of autonomy in proposing and progressing service change Work with relevant providers to develop ideas for service change that will bring better quality of care for people and/or more efficient, cost-effective services Lead promotion of designated work streams within the West Suffolk Alliance and neighbourhoods to ensure appropriate engagement and co-production with all interested stakeholders.

Act as key contact for West Suffolk Alliance partners, including providers and patient groups. Design care pathways and service specifications, in line with best evidence-based practice and patient choice. Responsible for the delivery of designated Live Well milestones and actions. Design monitoring arrangements for new and existing services and ensure that these are implemented and reviewed.

Detailed programme responsibilities will be assigned across the team, depending on service need priorities, capacity, knowledge, and skill set. You will be working across the Alliance transformation portfolio of work.
