An exciting opportunity has arisen in Suffolk and North East Essex Integrated Care System Cancer programme, for a Cancer Transformation Lead, hosted by WSFT and in conjunction with our partner organisation Macmillan Cancer support.

We have the ambition to bring together all services from all care domains to unify and integrate the patient’s cancer journey, promoting personalised care and the” what matters to me strategy”.

We are looking for an individual to fulfil this ambition by enabling the erosion of barriers between all key stakeholders across all domains to promote one seamless journey allowing the patient’s needs to be met in the situation best suited to them.

This role will be bringing organisation together mapping services and facilitating sharing of services through the development of network groups. The brokering of data sharing agreements and the development of an operational framework will be required to ensure the high standard of governance for our patient is maintained.

You will be working with partners across Suffolk and North East Essex to ensure new services and projects integrate successfully into the personalised care strategy to help drive through the transformation of cancer care for local patients. The role will require you to work closely with multiple organisations and partners as part of a team transforming cancer services for patients in our area.

The post holder will be passionate and have experience in project management. We are looking for someone who is willing to learn and develop within the role, so that they can progress on to solely leading the project. There will be milestones and identified development of the post holder that must be achieved with defined timescales. With the opportunity to develop into a 8a role. With the ultimate goal being that they will be able to lead on the successful change management and service re-design. Having achieved an good knowledge of NHS policy and specifically cancer standards. Developed excellent skills in managing relationships across multiple organisations and communication skills.

The post holder will have current experience of working within both an acute Trust and an ICB and will have lead on projects that involve multiply stakeholders across a variety of organisations

The purpose of this role is to support the Strategic Programmes Cancer Team to bring the CorePLUS5 principles to the forefront and to:
Understand how HNA’s and personalised care plans are being currently received and actioned whist working with individual organisations to strengthen delivery of plans in line with NHS digital guidance.

To work with the workforce that we have already positioned within primary care and the PCNS to ensure best practice is being shared across the ICS, with the promotion of HNAs, PCPs and CCRs being key,

To work with the Strategic Programmes Cancer Team on developing networks - bringing together like-minded individuals/ providers/service users from existing alliances to help the breaking down of barriers between organisations, promoting cohesive pathways and reducing inequality by looking at what is required to bring each organisation in line with the operational framework.

To work with the Strategic Programmes Cancer Team on mapping current support services - highlighting those areas of need/missing services or accessibility issues across all domains, whilst recognising duplication and fostering relationships between all organisations. Those service areas found to be lacking will become part of the work directed by the steering group.
