The North West London Remote Patient Monitoring Team are advertising for a matron for our virtual wards and remote patient monitoring hub, based at St Charles Centre for Health and Wellbeing, in Ladbroke Grove.

The remote patient monitoring hub undertakes monitoring of patients from across North West London acute hospitals in a single team. We cover a range of pathways, such as COVID, COPD, Heart Failure, IBD, maternity and general cardiac. Currently we have 21 virtual wards live with a range of SOPs that guide our clinical reviews and we are in the process of developing more pathways.

We are looking for an enthusiastic leader who is ready for a new challenge in a very different setting. Evidence of strong assessment skills and significant clinical experience in acute care are essential, as well as management experience at band 7 level or higher. The post holder will lead a team of band 7 nurses who are responsible for the day to day operational running of the hub. The post holder will be responsible for ensuring the provision of safe and effective care, excellent patient experience and staff wellbeing. This is a fast growing team so experience of staff development and HR processes is required.
Whilst this is a virtual environment, patient contact remains a key facet of the job. The hub is supported by a network of multi professional teams across all four of our acute organisations, and opportunities will also be available to keep up or develop patient facing clinical skills.

Remote monitoring and virtual wards are an exciting area of developing practice for nursing staff, and this post support the development across North West London. Virtual wards are a substitute for in hospital care, and are being established across the country, this is a great opportunity to be part of a move away from traditional models of care, towards a more sustainable future within the NHS.

Quality and safety of Nursing Care

**MAIN TASKS AND RESPONSIBILITIES**:

1. Clinical Responsibilities

1.1 Ensure high standards of patient care at all times challenging and ensuring others challenge poor practise, in accordance with the NMC Professional Code of Conduct.

1.2 In collaboration with senior members of the Divisional team provide assurance that the standards set out in the relevant Nursing & Midwifery strategy and direction of Nursing & Midwifery for the Trust are delivered at local level.

1.3 Ensure local plans support all clinical quality external accreditation, for example Care Quality Commission (CQC) essential standards, Safety Thermometer, Commissioning for Quality and Innovation (CQUIN), Quality Accounts, NHS Litigation Authority.

1.4 Ensure agreed systems and process to consistently provide safe, effective high quality patient care and embed a culture of improvement, focusing on the Trust’s harm free indicators, measuring practice and using variations in the data to develop local improvement projects are in place on the ward.

1.5 Through visible clinical leadership ensure that the delivery of kind, compassionate and respectful care takes place, and that the ward team develop relationships with patients that involve better listening and decision making; ‘no decision without me’

1.6 Act as the patient’s advocate when appropriate, when informed decisions may lead to choices being made concerning treatment options.

1.7 Ensure that the delivery of care meets the individual needs of all patients, and considers inclusivity and personalisation.

1.10 Ensure the maintenance of professional contemporaneous records at all times, document all actions and interventions in a clear and honest and timely manner in line with Trust policy.

1.11 Develop effective teamwork, to ensure patients are regularly reviewed as part of regular ward rounds, to include ongoing care plans especially for nights, weekend and bank holiday periods.

1.13 Using professional judgement be prepared to intercede and act as patient advocate to clarify prescribed plans of care.

1.14 Deal with concerns raised by patients and visitors in a proactive manner, review patient feedback results and update the ward’s patient experience improvement plans, as needed.

Ensure good public and patient involvement and that user views are taken into account in the planning and monitoring of services through FFT feedback, complaints and compliments.

1.15 Champion a safeguarding culture, which meets the needs of people with mental health problems and those with issues involving mental capacity
