The Buckinghamshire Patient Flow team are seeking to recruit a Clinical Patient Flow lead to join our team on a permanent basis.

The successful applicant will be an experienced mental health clinician with a good working knowledge of inpatient and community mental health teams and their thresholds.

The role involves supporting successful inpatient mental health admission in Buckinghamshire, including identifying barriers to discharge and supporting teams to overcome these.
- Working collaboratively across community, inpatient and urgent care services to support with patient flow, facilitate opportunities to prevent admission where clinically appropriate and supporting referring teams to develop clear purposes of admission and treatment goals
- Oversees patient journey from point of bed request to discharge
- Support with triaging all new bed requests and gathering information to understand the patient’s purpose of admission, likely length of stay, early detection of non-clinical barriers to discharge
- Work alongside inpatient and community teams to support with unblocking barriers to discharge, facilitating timely admission and discharges from hospital. Attend rapid reviews, ward rounds, patient flow calls and complex discharge CPA’s
- Support the Trust initiative to achieve zero inappropriate usage through effective flow through community and inpatient services
- Share the chairing of the twice daily patient flow calls with support from manager
- Contribute to service improvement and transformation work designed to improve patient and carer experience through improving access to local, timely care and treatment.

You will be based at The Whiteleaf Centre with expectations to visit the local wards when required.
