**Are you an Registered Nurse with community based complex care experience looking for your next challenge?**

Our client is national provider of community based complex care, and are looking to add more superstars to their team. You will be responsible for a caseload of clients with complex care needs working with the care teams to deliver outstanding client outcomes in the community.

**Key Responsibilities**:

- Undertaking initial client assessments and setting up initial care plans and risk assessments
- Undertaking clinical competency training and sign off with support staff
- Providing ongoing supervision of staff teams
- Ongoing support and management of service user's care packages
- Ensuring all packages of care remain safe and effective within clinical and CQC requirements
- Take part in an on call rota to provide clinical guidance to care teams

To qualify for the role you will need an active NMC PIN and your own transport.

**Salary**:
£43k salary plus £3k car allowance taking the package to £46k

**Job Types**: Full-time, Permanent

Pay: Up to £46,000.00 per year
