**Main duties of the job**

The role of the Social Prescribing Link Worker is to provide social support to patients across the PCN. You will work individually with patients, taking the time to listen and understand what matters to them, and supporting them to access additional services, groups or social activities.

The postholder will need to confidently evidence and utilise key skills such as effective communication, ability to empower people to take control of their health and wellbeing through referral to ‘link workers’ who give time, focus on ‘what matters to me’ and take a holistic approach to an individual’s health and wellbeing, connecting people to diverse community groups and statutory services for practical and emotional support.

You will carry and manage a case load of patients, working independently within your remit to support patients. All social prescribers receive clinical supervision.

The postholder will work in collaboration with local divisions (including Divisional Boards) and the National Modality Business Support Team to deliver innovative ways of working to meet the strategic aims of the organisation.

The role is not a regular 9am-5pm job, the job requires innovation, flexibility, and commitment. The postholder will be required to work resourcefully as part of the team to ensure tasks are completed.
