**Purpose of the role**

The Health Coach supports patients to take control of their health and wellbeing by giving the patient’s time to focus on ‘what matters to me’. They consider the whole person in addressing existing issues and encouraging the proactive prevention of new illnesses, promoting lifestyle medicine approaches. Health coaching revolves around using coaching skills to support people with lower levels of patient activation to develop the knowledge, skills and confidence to manage their health and wellbeing, whilst also increasing their ability to access and utilise community sport offers.

Health coaches support and encourage the prevention of developing further illness, or the deterioration of existing long-term conditions. The role is varied and may include supporting self-management education, peer support, case management and facilitating group work. The approach will be to appreciate patients’ circumstances in a non-judgmental manner. This will result in the need for positive communication and negotiation skills that will support the patient personal choices.

**The post holder will**:

- Proactively contact and engage with clinical staff to encourage referrals and follow up referrals within a timely manner.
- Motivate, empower and encourage people to take positive action to improve their health and wellbeing, by supporting education around health conditions and lifestyle measures
- Develop trusting relationships by giving people time and focus on ‘what matters to me’.
- Take a holistic approach, based on the person’s priorities and the wider determinants of health.
- Co-produce a personalised support plan (action plan) to improve health and wellbeing, introducing or reconnecting people to community groups and statutory services.
- Manage and prioritise your own caseload, in accordance with the needs, priorities and any urgent support required by individuals.
- Accept referrals for people with health conditions (including common mental health conditions, obesity, diabetes, respiratory conditions, mobility issues and sensory impairment)
- Work with people in a supportive, holistic way focussing on strength-based assessment to address practical and psychological barriers, such as lack of transport, low confidence and social isolation, to co-produce a solution.
- Support people to establish and attain goals set by the person based on what is important to them
- Provide personalised support to individuals, their families and carers to take control of their wellbeing, live independently and improve their health outcomes.
- Refer people back to other health professionals/agencies, when the person has needs beyond the scope of the link worker role - e.g. when there is a mental health need requiring a qualified practitioner.
- Ensure all necessary data and information about Clients is recorded accurately and entered confidentially on EMIS or other NHS collection methods, with awareness of information governance best practice.
- Use recognised tools with patients to track improvements in their health and wellbeing using ONS scales of Wellbeing. Work with the Social Prescribing Lead and GP practice to review data on GP appointments and hospital admissions to track statistical improvements at the practice.
- Work with the social prescribers to connect clients to community-based activities which support their health and well-being.
- Work in partnership with local organisations to build a comprehensive directory of local resource to design and support health coaching and social prescribing. Ensure information is always up to date to enable effective and accurate signposting and linking of patients with services.
- Work in collaboration with the Social Prescribing Lead and key staff in the GP practices within the PCN, attending relevant meetings, becoming part of the wider network team, giving information and feedback on health coaching to ensure promotion of the service.
- Recruit volunteers to support the service locally. Engage with Patient Participation Groups, existing community groups, patients etc. and staff to promote volunteer opportunities such as peer support.
- To contribute to public health campaigns through advice or direct care
- Work closely with partners to ensure support for patients is complementary and people access the right service for their needs.
- Any other tasks and responsibilities that may be identified as necessary as the service evolves and develops
