The post holder will work within Lancashire & South Cumbria ICB Children & Young People Continuing Care Team to ensure delivery of the best possible care within available funding, representing “value for money,” to support the seamless implementation of the National Framework for Children and Young People's Continuing Care (CYPCC). Experience with mental health, learning disability or physical disability is required

The post holder will provide advice, screen referrals and co-ordinate assessments in alignment with the National Framework for Continuing Care and local policies, supporting an end to end Children and Young People's Continuing Care service. The post holder will process, and quality assure referrals/checklists and decision support tools.

The post holder will work as part of the CYPCC team to provide a case management approach by working with specialists across the fields of physical disability, learning disability, mental health, and end of life needs, to ensure the child or young person receives quality health care and achieves the best outcomes for them.

The post holder will influence best practice by providing professional clinical advice on a range of physical, learning disability and mental health nursing issues, which will require the individual to work in collaboration with partners and other professionals to ensure children and young people’s’ health care needs are met appropriately, safely and effectively.
- Operate effectively in a flexible and increasingly demanding environment proactively engaging with internal and external clinical and non-clinical colleagues within the ICB, Local Authorities and partner organisations, care providers, NHS England and CQC.
- Communicate and present highly complex, sensitive and contentious information to a wide range of internal and external stakeholders in a formal setting, with the ability to deal with potentially aggressive and antagonistic conflict situations.
- Work and engage constructively with internal and external stakeholders on a range of contentious quality and safety issues in order to ensure that services commissioned by the ICB are provided effectively and safely, and to the appropriate level of quality. Scrutinise and challenge providers to ensure the ICB fulfils its duty to secure continuous improvement in the quality of commissioned care home and domiciliary services.
- Develop and maintain key relationships internally and externally, including national networks, in order to provide advice and support to the ICB based on evidence and national guidance, including NHS England CHC Network, Personal Health Budget Network and CQC/Ofsted meetings.

1. Key Responsibilities
- To have a working knowledge of the National Framework for Children and Young People's Continuing Care and maintain relevant and up to date knowledge and skills.
- To ensure every assessment performed is completed holistically on an individual basis within a multi-agency arena. This includes managing referrals, assessments and the process thereafter for children and young people who have a physical disability, mental health, learning disability or end of life needs, in the community (at home/residential/hospice), acute setting (hospital) or other setting.
- To manage and take responsibility of Continuing Care Assessments, making individual recommendations as an outcome of assessment within guidance of the Children’s National Framework and local policy.
- To undertake joint assessments in conjunction with family/carers and with other professionals from health and social care and other agencies, to ensure the health needs of patients are identified and appropriately met within the remit of the National Framework guidance and local policies/processes.
- To work within agreed systems and processes to secure best value for money packages, recognising current financial situation of the ICB, meeting clinical needs, working within the current service commissioning process.
- To promote physical and mental well-being and maximise child/young person family/carer empowerment in the management of their care. Where necessary ensure individuals have access to an advocate
- To ensure the voice of the child and young person is listened to
