The post holder will act as a Senior Children's Assessor and exemplar for best practice within the Children and Young People’s Continuing Care Team (CYPCC) for children and young people with complex physical health, mental health and learning disability needs.

The post holder will work within Lancashire & South Cumbria ICB Children & Young People Continuing Care Team to ensure delivery of best possible care within available funding, representing “value for money”, to support the seamless implementation of the National Framework for Children and Young People's Continuing Care (CYPCC). This role includes contribution to the development of the service, training programmes and facilitating others learning.

The post holder will provide advice, screen referrals and co-ordinate assessments in alignment with the National Framework for Continuing Care and local policies, supporting an end to end Children and Young People's Continuing Care service. The post holder will process, and quality assure referrals/checklists and decision support tools.

The post holder will work as part of the CYPCC team to provide a case management approach by working with specialists across the fields of physical disability, learning disability, mental health, and end of life needs, to ensure the child or young person receives quality health care to achieve the best outcomes for them.
- Lead the CYPCC team in providing in-depth support and guidance for those cases that cause increased challenge, requiring a skillset to manage complex situations which can include legal challenge, court proceedings, complex disputes/appeals/complaints.
- Lead the CYPCC team in provision of assessment, case management and review process for children with complex healthcare needs related to Continuing Care, Complex Cases children and young people’s cases including physical, mental health needs, learning disabilities or autism. Inclusive of liaison where required, with the child’s family/parents/carers.
- Gain assurances of safe and appropriate provision within residential or therapeutic placement, via multi-agency review process. Working with specialist health and Local Authority colleagues to assess continued suitability and appropriateness of placement, and gaining assurances that placement is liaising with NHS core and specialist services as required.Lancashire & South Cumbria ICB is a fully-authorised ICB responsible for the full range of ICB statutory duties and powers. - Contribute to a continuous review process in line with the Looked After Children health review process, Special Educational Needs and Disability.
- Where necessary provide professional governance oversight of Personal Health Budget development, implementation and subsequent review process, alongside those responsible for Personal Health Budget delivery.
- Identify and recommend service improvements, aiding or enhancing provision of care & ensure young people are effectively safeguarded.
- Take an active role and where required lead on the development of policies, procedures and protocols for enhancing professional practice and workforce development within the area of responsibility.
- Provide and lead on training within specialist area of responsibility as appropriate, and support CYPCC team members to carry out training as required.
- Provide advice and support and be an exemplar of best practice to relevant service transformation and redesign projects as required.
- Develop and maintain collaborative working relationships with stakeholders and partners across the local area health footprint and social care community, participating in network meetings where appropriate. Use these meetings and relationships to influence the development and implementation of policy and initiatives. Providing support and guidance to junior staff through these initiatives.
- Contribute to and collate activity and performance data together with specialist analysis and commentary, lead on delivery to ICB performance reports as required.
