Senior Children's Assessor

To ensure the health care plan involves satisfactory outcomes for children and young people including safe discharge from acute/community provision, transition between placements and facilitates continuity of care. Contribute to a continuous review process in line with the Looked After Children health review process, Special Educational Needs and Disability. Where necessary provide professional governance oversight of Personal Health Budget development, implementation and subsequent review process, alongside those responsible for Personal Health Budget delivery. Identify and recommend service improvements, aiding or enhancing provision of care & ensure young people are effectively safeguarded.
Take an active role and where required lead on the development of policies, procedures and protocols for enhancing professional practice and workforce development within the area of responsibility. Provide and lead on training within specialist area of responsibility as appropriate, and support CYPCC team members to carry out training as required. Provide advice and support and be an exemplar of best practice to relevant service transformation and redesign projects as required. Develop and maintain collaborative working relationships with stakeholders and partners across the local area health footprint and social care community, participating in network meetings where appropriate.
Use these meetings and relationships to influence the development and implementation of policy and initiatives. Providing support and guidance to junior staff through these initiatives. Contribute to and collate activity and performance data together with specialist analysis and commentary, lead on delivery to ICB performance reports as required. Maintain effective communication with patients and carers/advocates and professionals across health and social services.
Take the lead on and support junior staff where required through difficult and challenging scenarios and situations. Contribute to formal and informal education and training to health, social care and education colleagues, independent sector, voluntary sector. Responsible for own professional development and be actively involved in professional and multi-agency initiatives. Responsible for ensuring joint contractual arrangements and reporting are in place for each child or young person, in liaison with the relevant procurement/contracting, income and charging departments within ICB and Local Authorities.
Access up-to-date research evidence to secure cost and service improvements from alternative methods of operation, through new goods and services or re-engineering existing processes. Key Working Relationships Operate effectively in a flexible and increasingly demanding environment proactively engaging with internal and external clinical and non-clinical colleagues within the ICB, Local Authorities and partner organisations, care providers, NHS England and CQC. Communicate and present highly complex, sensitive and contentious information to a wide range of internal and external stakeholders in a formal setting, with the ability to deal with potentially aggressive and antagonistic conflict situations. Work and engage constructively with internal and external stakeholders on a range of contentious quality and safety issues in order to ensure that services commissioned by the ICB are provided effectively and safely, and to the appropriate level of quality.
Scrutinise and challenge providers to ensure the ICB fulfils its duty to secure continuous improvement in the quality of commissioned care home and domiciliary services. Develop and maintain key relationships internally and externally, including national networks, in order to provide advice and support to the ICB based on evidence and national guidance, including NHS England CHC Network, Personal Health Budget Network and CQC/Ofsted meetings. Clinical: Support the CYPCC Clinical Lead in managing and coordinating the Childrens service. Work alongside the multi-agency team with regards to the coordination of the discharge from hospital environments/acute/residential settings of children with complex health needs, liaising with specialist health teams, community nursing teams, education, voluntary agencies, social services, children with disabilities team, NHS and independent nursing and care agencies to address the needs of the child and young person.
Advise the teams involved in the requirement and development of appropriate care plans, risk assessments and contingency planning to meet the complex needs of the children and young people. Liaison with procurement/commissioning team to successfully navigate the process to procure bespoke commissioned services to meet the needs of the child or young person.
