Our client is a very reputable, family run domiciliary home care provider, well established, that seeks a Field Care Supervisor covering care around Bournemouth and Poole areas.
**What is the job of a Care Coordinator?**
A **Care Coordinator** forms a close relationship leading a small team or care workers. Here is a brief breakdown of the role
- 50 /50 Field based and office based
- Weekly rostering and all the attributing areas to this responsibility
- First got go to point for new and existing clients and making relevant decisions
- All care needs are suitably covered
- A close relationship with Care Management to implement the care package and identifying needs to the ideal care worker
- As required to undertake client assessments and reviews alongside the care manager
- Upholding CQC regulations and to continually stay updated on any CQC regulations updates
- Providing excellent level of care standards, office duties, communications, admin.
- Equally undertaking care work in the field alongside office role.
**What we are looking for in a **Care Coordinator**?**
Experience in daily charts, medical administration and good computer knowledge all needed.
A driving license and transport a must.
