An exciting opportunity has arisen for a dynamic and experienced Neonatal Nurse to join our supportive and friendly team working on a tertiary level three Neonatal Unit. The Discharge Co-ordinator Role assists, facilitates, supports and actively participates in overseeing the management of the discharge process supporting both junior and senior staff. Facilitating early parental involvement and education. Coordinate discharge planning, liaise and communicate with Safeguarding, MDT, specialist community services and local agencies for a safe, smooth and timely discharge.
1. To lead on improving the care & discharge process for infants on NICU
2. Establish and maintain interdepartmental communication links to support the transition from hospital to home.
3. Participate in multi-disciplinary team meetings, providing data to support decision making.
4. Use and develop own skills and knowledge and provide information to others to others to help develop pathways.
5. Monitor and maintain health, safety and security of self and others in own work a
6. Assist in maintaining and developing service
7. Ensure own actions help to maintain quality and patient experience.
8. Ensure own actions support equality, diversity and rig
9. Assist in planning infants discharge in line with procedures, guidelines and agreed pathways.
10. Assist in planning, delivering and evaluating programmes to improve patient and family experience.
