We are looking to recruit a Permanent part time dynamic and experienced Band 6 RMN to join our Older Peoples Liaison Mental Health Service. The service aims to provide older people in Oldham with mental health problems, care and support closer to home, avoiding admissions to secondary services and long-term care wherever possible, and sits between primary care services and secondary services . The service supports patients referred either by the acute hospital or GP via living well hub and community-based cluster teams with a suspected or actual mental health problem. The team is comprised of a group of Band 6 RMN's and you would work as part of the MDT and undertake duties within the Older People's Liaison Team including assessment, care planning, triage and interventions as specified within individual care plans.


The current postholder will leave the service at the beginning of June at which time the successful candidate will be able to start immediately
An exciting opportunity has arisen for passionate and dedicated RMNs to join the Community MH liaison service within Oldham as services gradually move to a neighbourhood cluster model. As the service model is changing this will mean that there may be changes which affect this post - it will remain however a permanent post


This service will provide mental health expertise within the multidisciplinary cluster teams and community enablement services. You will be working with a wide range of colleagues from health and social care and other statutory and voluntary agencies to support individuals in the community or those within the hospital discharge, enablement and intermediate care pathway.


The service operates Monday to Friday and you will work predominantly with, and have a passion for supporting older adults. You will be providing full mental health and risk assessments and short term interventions with individuals in collaboration with their support network. You will be working within the multidisciplinary team with a strong focus on improving quality of life and preventing unnecessary admissions to hospital and long term care. Where required you will signpost people in to secondary care mental health services.


This is an exciting opportunity to join a small dedicated team of band 6 nurses who provide nursing care and treatment to patients living in the community.


You will have the opportunity to work closely with other teams in the older peoples services across the Trust and in social services.

The service willoperate and be co-located within borough cluster care teams and will operate Monday to Friday 9-5.To provide specialist nursing advice and assessment skills in conjunction with health, community and social care staff in order to fully develop appropriate interventionsand treatment plans for people with mental health problems.To carry out comprehensive risk assessments of mental health needs and have knowledge and experience of risks within the community.To supervise the work of junior staff and provide clinical advice and support to the team.It is expected that the post holder will deliver care in accordance with all Pennine Care NHS Foundation Trust policies and procedures, the Mental Health Act, Mental Capacity Act and community care legislation. Main Duties and Responsibilities Clinical WorkTo work across cluster teams and the discharge / enablement pathway to provide specialist mental health nursing advice and a joint assessment serviceTriage all referrals that are made to the service and allocate clinical priority.To work in close liaison with staff within health and social care in upholding and implementing clinical standards for the care of people with mental health needs within the community settingTo carry out a thorough assessment of the service users current mental health needs, including risk assessment, psychological needs and cognitive functioningTo demonstrate the ability to make decisions in relation to the clinical management of individual patients and make critical clinical judgements in relation to mental health and care provision within the community setting.To be able to assess and manage clients presenting with co-morbidities of physical and mental health needs including the impact of medication.To model appropriate methods of communication and care delivery to staff within the cluster teamsContribute to making decisions regarding client care in consultation other members of the multi-disciplinary team.To undertake short term nursing interventions and/or co-ordinate transition of care to other specialist support.To utilise counselling skills when communicating with clients and carers in relation to assessment, diagnosis and treatment plan.To identify the need for protection in line with Safeguarding Adults Policy and comply with local procedures.To evaluate systematically and record, outcomes of mental health treatment plans, adapting care on the basis of these outcomes.To involve family and carers in the assessment and care planning with service users consent.To provide education and appropriate self-help resources to patients specific to individual needs.To provide advice and information to non-mental health trained social care staff regarding the management of patients with mental health problems..To observe and practice within the legal requirements of the Mental Health Act 1983 and Mental Capacity Act.To contribute to audits as required,To provide relevant data as required in line with performance monitoring targets.Education and TrainingTo contribute to the on-going training and development of non- mental health trained social care staff with regard to mental health issues.To have effective presentation skills in order to positively promote the social care liaison teamTo provide mentorship, education and training to learners from a range of professional backgrounds.To attend mandatory and other training as identified as relevant to the post and necessary to ensure safe effective practiceService DevelopmentTo participate in monitoring and evaluation of the effectiveness of service delivery and contribute to developments relevant to mental health services.To deal appropriately with clinical incidents, complaints etc from patients, relatives and members of the public, via line manager, in accordance with Trust policy.
Registered Mental Health Nurse (RMN)
Short courses and experience to post graduate diploma level.
Non-medical prescribing qualification
Significant post registration experience at Band 5 or above.
Experience of working with Older People within community settings.
Experience of working with individuals who present to mental health services in crisis and have symptoms of mental health illness
Experience of using safeguarding adult procedures

Experience of working with working age adults within a community setting

Understanding of current mental health legislation, vulnerable adult and safeguarding procedures
Knowledge of the mental health act and mental capacity act.
Awareness of NICE Guidance in the treatment of mental health problems and knowledge of treatments and interventions appropriate for individuals with mental health needs
Understanding of confidentiality and data protection issues
Knowledge of mental health assessment and risk assessment / management processes
Comprehensive knowledge regarding the impact of physical health disorders on mental health
Comprehensive mental health and risk assessment skills.
Ability to supervise and direct the work of junior staff
Ability to construct effective packages of care and risk management plans
Ability to execute a range of mental health assessment tools
Ability to work autonomously in a community setting
To have the ability to devise and present training packages
Must be flexible and able to work within a 7 day service, including bank holidays
Use of a car or access to a means of mobility to travel across the Trust footprint in line with service needs
