Hywel Dda University Health Board has a new, exciting opportunity for motivated, driven and passionate individuals who would like to join the Physiotherapy and Occupational Therapy workforce here in the stunning county of Pembrokeshire.

With the development Therapy Assistant Practitioner posts, successful applicants will deliver rehabilitation for patients identified by Physiotherapists or Occupational Therapists. The role will be within the frailty service working at the front door and on the frailty assessment unit. The unit aims to reduce the risk deconditioning by trying to turn patients around with 72hours if possible.

These new roles will support the Frailty pathway and "Home First" approach, supporting rapid turnaround of patients and flow of patients out of the Hospital.

We are looking for people with enthusiasm and motivation to work constructively as part of a multidisciplinary team and wider network in the County, including the Frailty Team, Urgent & Intermediate Care and the wider therapy teams in Pembrokeshire.

Successful applicants will have experience of using highly skilled handling techniques to facilitate patient movement, and/or rehabilitation. You will be managed and supported by a Physiotherapist and Occupational Therapist to give you comprehensive supervision structure. You should be able to demonstrate clinical reasoning skills, to enable you to assess and treat patients within protocols, without immediate direct supervision. You will work with others in the team, i.e. dietitians, to deliver a person focussed service.

You will be supported to work towards the Level 4 Therapies diploma whilst practicing. The pandemic has taught us more than ever the importance of work life balance and we encourage remote working when appropriate and flexible working arrangements.

This is an exciting opportunity for a driven individual, passionate about the value of therapy to play a big role in transforming our services across organisations for the people of Pembrokeshire and future generations.

You will need to be able to work across 7 days as part of a rota and have the ability to travel to support patients in the community setting.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales. We do this in partnership with our three local authorities and public, private and third sector colleagues, including our volunteers, through:
**Four main hospitals**: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;
**Seven community hospitals**: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;
48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;
Numerous locations providing mental health and learning disabilities services;
Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales
