About the Role

We are seeking an experienced Healthcare Support Worker to join our Community Diabetes Team in Pembrokeshire as a Diabetes Assistant Practitioner. This is a unique opportunity to be part of an exciting new project aimed at improving access to diabetes surveillance, care, and education for those who are housebound or not able to access primary care interventions in our locality.

Key Responsibilities
  • Work with the CNS team and primary and community colleagues to expedite proactive foot checks and foot health surveillance.
  • Perform ankle-brachial pressure index (ABPI) testing, deliver smoking cessation advice, diabetic education, and liaise with diabetic podiatry, vascular, and retinopathy services.
  • Monitor and record vital signs, weight checks, and perform blood glucose monitoring.
  • Support an RN with wound care and provide non-complex wound care.
Requirements
  • Level 4 vocational qualification in a relevant healthcare-associated subject or Certificate in Healthcare Support Worker Education.
  • GCSE Maths and English Grade C or above or equivalent level 2 or above essential skills qualifications.
  • Evidence of continuing personal development relevant to the role in healthcare.
  • Able to demonstrate understanding of the HCSW Code of Conduct, All Wales Delegation guidelines, and the All Wales Band 4 Assistant Practitioner Governance Framework.
Desirable Qualifications and Experience
  • Local knowledge of statutory and voluntary services.
  • Teaching or assessment qualification.
  • Experience in a range of clinical skills including palliative care.
