We are seeking an experienced Diabetes Assistant Practitioner to join our Community Diabetes Team in Pembrokeshire. As a key member of our team, you will play a vital role in improving access to diabetes surveillance, care, and education for those who are housebound or unable to access primary care interventions.
Key Responsibilities- Work with the CNS team and primary and community colleagues to expedite proactive foot checks and foot health surveillance.
- Perform ankle-brachial pressure index (ABPI) testing, deliver smoking cessation advice, diabetic education, and liaise with diabetic podiatry, vascular, and retinopathy services.
- Monitor and record vital observations and interpret results.
- Review risk assessments and monitor and interpret blood glucose levels.
- Support an RN with wound care and provide non-complex wound care.
- Level 4 vocational qualification in a relevant healthcare-associated subject or Certificate in Healthcare Support Worker Education.
- GCSE Maths and English Grade C or above or equivalent level 2 or above essential skills qualifications.
- Evidence of continuing personal development relevant to the role in healthcare.
- Able to demonstrate understanding of the HCSW Code of Conduct, All Wales Delegation guidelines, and the All Wales Band 4 Assistant Practitioner Governance Framework.
- Local knowledge of statutory and voluntary services.
- Teaching or assessment qualification.
- Experience in a range of clinical skills, including palliative care.
- Administration of medicines via oral, topical, SC, IM routes.
- Skills in non-complex wound care.
