Diabetes Assistant Practitioner

This is a fixed-term vacancy for a Diabetes Assistant Practitioner to join the Community Diabetes Team in Pembrokeshire. The post holder will be an integral part of the North Pembrokeshire Diabetes cluster project, aimed at improving access to diabetes surveillance, care, and education for those who are housebound or unable to access primary care interventions.
Main DutiesThe Diabetes Assistant Practitioner will work with the CNS team and primary and community colleagues to expedite proactive foot checks and foot health surveillance, phlebotomy, recording of vital signs, weight checks, perform ankle-brachial pressure index (ABPI) testing, deliver smoking cessation advice, diabetic education, and liaise with diabetic podiatry, vascular, and retinopathy services.
About the RoleThis is an exciting opportunity for an experienced healthcare support worker with a level 4 certificate in Health Care Studies to join our team. The successful candidate will have significant and recent experience of providing relevant clinical care, interpersonal and communication skills, and the ability to manage and diffuse challenging or threatening situations.
Person SpecificationThe ideal candidate will have a level 4 vocational qualification in a relevant health care associated subject, GCSE Maths and English Grade C or above, and evidence of continuing personal development relevant to the role in health care. They will also be able to demonstrate understanding of the HCSW Code of Conduct, All Wales Delegation guidelines, and the All Wales Band 4 Assistant Practitioner Governance Framework.
Language SkillsThe ability to communicate in Welsh is desirable for this post. The Health Board offers a variety of learning options and staff support to help you meet these minimal desirable requirements during the course of your employment with us.
