**Job Summary**:
The Care Coordinator is a full-time on-site role located in Nottinghamshire. As a Care Coordinator, you will be responsible for the overall management and scheduling of staff and client rotas and will participate in the on-call rota. You will be responsible for supporting the Management Team with Business Growth and ensuring we are always delivering the highest possible standard of care.

When necessary, the Care Coordinator will undertake the completion of relevant assessments and updating the care plans for our Service Users. In addition, you will be responsible for liaising with the Human Resource and Admin Manager with completing any day-to-day tasks including the coordination of staffing

We are looking for someone who is willing to lead by example and go the extra mile for their team, creating a work atmosphere that promotes a high quality of work life. You will be flexible to the needs of the service, have the ability to multi-task and work well under pressure.

**Key Responsibilities**:
Managing staff across a live rota

Coordinate and allocate shifts to ensure all shifts are filled in advance

Ensure adequately matched carers, colleagues and clients are met

Efficiently manage multiple tasks and responsibilities.

Undertake and record supervisions and conduct spot checks as required.

Willingness to work in various service locations, including supported living facilities.

Develop relationships with the care team

Partake in on call duties

Recording of care team weekly timesheets.

Undertake Shadowing of staff or work in the field if needed.

Work closely with the Health Services Manager and may assist with Social Care Tendering processes.

**Qualifications**:
Bachelor's degree in a related field (e.g., social work, nursing, healthcare administration).

Proven experience in a care coordination role.

Knowledge of social care tender processes.

Valid driving license with access to own transportation.

Proficiency in documentation and record-keeping.

Familiarity with relevant laws and regulations in healthcare and social services.

**To be considered for this role you will need**:
Previous experience of working within a healthcare setting

The ability to work both independently and as part of a team

To be pro-active, think fast and problem solve under pressure

Have strong organization skills and excellent attention to detail

Great written and verbal communication skills

Be compassionate, empathetic and show patience in all you do

Experience of using technical platforms such as: Care Planner, excel, outlook.

**Job Type**: Part-time

**Salary**: £14,500.00-£17,500.00 per year

Expected hours: 24 per week
