We are currently recruiting for a Community Link Facilitator who will work with both Hartley and Jubilee ward, but is based in the Occupational Therapy department. You be under guidance from the Occupational therapist and have support from the OT team.
Using a person-centred approach, you will be supporting people with their community leave. Each patient is entitled to leave, and it is often a key part of their care and rehabilitation. Leave comes in many forms, from the functional to the fun. While one day might involve a routine hospital appointment, another may be visiting a leisure centre, library or the local shops.
You will help patients build the skills they need in order to move forward in their lives, as well as encouraging their independence. You will work with a diverse group of patients with varying levels of needs and cognitive functioning. A flexible approach and willingness to learn is therefore vital in this role. You will find yourself in a friendly multi-disciplinary team, and have access to full training provided by Priory.
The role is 8.30am-4.30pm over a rolling 5 days a week which can include weekends. No nights are required.
All training for this role will be provided.
