**Main duties and responsibilities**: - Take referrals from GP practices within Primary Care Networks, providing telephone consultations with patients (and possibly carers etc) and occasional community-based appointments. i.e. housebound patients, community service events. - Monitor referrals and assess appropriateness for the Social Prescribing Service.
- It is vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals, when what the person needs is beyond the scope of the link worker role. - Make contact with new referrals, ensuring they have been provided information about the social prescribing service and provide additional information when requested. - Develop relationships by giving people time and focus on 'what matters to me'. Taking holistic approach, based on persons priorities and wider determinants of health.
- Signposting patients to support in the local community and undertaking onward referrals where appropriate. - Be creative in finding solutions that harness assets and resources which are already available. - Refer back with recommendations if the referral is inappropriate or best placed for an alternative service or provision. - Build relationships with staff in GP practices within the local Primary Care Network (PCN), attending relevant meetings, becoming part of the wider network team, giving information and feedback on social prescribing to promote the social prescribing service.
- Provide personalised support to individuals, their families and carers to take control of their wellbeing, live independently and improve their health outcomes. Supporting them with issues such as debt, housing, employment, loneliness and caring responsibilities. - Integrate into and form part of General Practice/Primary Care Network teams to provide the support needed in those communities they serve. - Undertake system reporting duties to gather and analyse data providing recommendations.
- Liaise and communicate with Patients, Carers, Advocates, Health and Social Care professionals, voluntary sector and stakeholders. - Work sensitively with people, their families and carers to capture key information, enabling tracking referrals into the service and the impact of social prescribing. - Any other duties commensurate with the role as advised by your line manager. Date processing and recording: - Using systm1 and EMIS to record information and enable tracking of the impact of social prescribing on health and wellbeing.
- Encourage patients to provide feedback and share their stories about the impact of social prescribing on their lives. - Follow agreed and set standard operating procedures to record data and demonstrate clear outcomes and impact. - Adhere to GDPR and data protection requirements at all times. Keeping up to date with training and identifying potential breeches quickly and appropriately.
- Production of relevant reports - Supporting the social prescribing team to record local community groups and services available for patients. - Keeping patient confidentiality at the heart of what we do only access a persons information in the legitimate execution of your duties. Professional development: - Work with your line manager to undertake continual personal and professional development, taking an active role in reviewing and developing the post. - Undertake relevant training as required.Continually update own knowledge and skills within the job role.
- Engage in developing professional reationships with the wider team. - Attend supervision, annual appraisals and team meetings. - Demonstrate effective, professional and respectful communication with patients and staff. - Adhere to organisational policies and procedures including confidentiality, professional boundaries, lone working, safeguarding and information governance.
