**The Role**

A new and exciting project is being developed in Calderdale between Childrens Social care and Together for Housing.

To support this project we are recruiting 2 Project workers who will work along side the Project Manager.

The project will work intensively with approximately 8 children in care and care leavers at any one time,

This is a fantastic opportunity to be involved from the very beginning of implementing the project and to be able to make a significant difference to the outcomes of young people.

The role will involve supporting young people aged between 16 and 18 years old with preparation for independence and their Housing pathway.

Young people leaving care can be at risk of poor outcomes as they transition to independent living without the support of close family. Many young people struggle to live on their own, facing increased isolation, higher risks of mental health problems, homelessness, and becoming NEET or being involved in the criminal justice system.

The aim is to be able to deliver a range of Independence based initiatives and programmes and therapeutic approaches, based on the Dialectical Behavioural Therapy model, to work with individuals, groups, families, appropriate to the needs of the young people in the Service, and within the least restrictive environment possible. This will increase the sense of engagement, confidence, self-esteem, and empowerment and to contribute to positive outcomes for those young people.
