This post has been created utilising investment from Calderdale Integrated Care Board (ICB) which has been provided to invest in local interventions that will help improve the uptake of annual diabetic foot screening in Calderdale. The investment aims to help improve compliance in Calderdale to bring it in line with the national average, aiming to improve the clinical outcomes for patients through supporting access to the necessary services.

The post will support the Podiatry Team Lead for Calderdale and Huddersfield NHS Trust with the diabetic foot screening service, and work in partnership with Calderdale Primary Care Networks (PCN) to establish initiatives across these organisations to support improving access and optimising compliance with diabetic foot screening in Calderdale.

The post holder will be responsible for leading the project to improve diabetic screening in Calderdale. Diabetic foot screening is currently provided in General Practices by podiatry assistants who are competent to provide diabetic foot screening. They identify any concerns in a timely way and educate patients in relation to diabetes and foot care.
- To work with a range of people across the ICB who have responsibility for improving the experience and clinical outcome for diabetic patients.
- To work in collaboration with the Outpatient Therapy services manager and Podiatry Team lead to co-ordinate, design and deliver the project specifically on the provision and optimisation of diabetic foot screening in Calderdale.
- To identify the current barriers to accessing diabetic foot screening for Calderdale patients.
- To work with PCNs on improving communication around the planning and optimisation of the service.
- To establish a model for year-on-year diabetic foot screening service planning to ensure long term compliance above national average in Calderdale.
- To establish the scale of health inequalities in compliance with foot checks and implement strategies to address health inequalities where they are identified.

This post will work in the Community Division at CHFT which predominantly provides support to the Calderdale area.

The Community Division is a forward looking team which engages in developing services in the Health economy.
- Work with a range of people across the ICB who have responsibility for improving the experience and clinical outcomes for diabetic patients.
- Work in collaboration with the Outpatient Therapy services manager and Podiatry Team lead to co-ordinate, design and deliver the project specifically on the provision and optimisation of diabetic foot screening in Calderdale.
- Identify the current barriers to accessing diabetic foot screening for Calderdale patients.
- Work with PCNs on improving communication around the planning and optimisation of the service.
- Integrate with other diabetes initiatives and services, such as the diabetes health action plan, with the aim of having consistent communication, education, and provision across Calderdale.
- Establish the scale of health inequalities in compliance with foot checks and implement strategies to address health inequalities where they are identified.
- Review baseline data and intelligence, including current foot screening numbers, slot utilisation, demand and capacity, and patient satisfaction.
- Contribute to the development of a gap analysis and training needs analysis.
- Engage with a range of current multidisciplinary staff and support workers to gather insights and intelligence
- Collate examples of good practice that could be shared.
- Review possible technological solutions to support with foot screening compliance.
- Produce reports to ICB diabetes programme manager and CHFT community divisional board on activities, outputs and outcomes of the project.
- Develop a project plan with clear objectives and time frames to deliver key performance indicators and to raise awareness of risks which threaten to prevent these from being achieved.
- Ensure project issues are appropriately maintained.
- Maintain the ability to travel between sites and willingness to meet with staff on a face to face as well as in virtual forums where deemed appropriate to the situation
- Report regularly to the line manager to ensure there is strong governance.
- Deliver training and education where appropriate.
- To share learning from other services and organisations where good examples are noted of ways in which annual diabetic screening is optimised.
- Support audit and evaluations pertaining to the project
- Broaden research and development skills through participation in relevant local audit and evaluation projects
- Ensure practice is evidence based.
