**Mental Health Nurse**

We are looking for an exceptional, dynamic & innovative Mental Health Nurse to contribute to an effective occupational therapy service to meet the needs of our Client.

**Job Purpose**:
Based in the Leeds area, the role of the Liaison and Diversion Mental Health Nurse is an integral role within the team and involves identifying vulnerable adults and young people in contact with the criminal justice system, providing comprehensive assessments,preparing and presenting reports for Courts, linking with mental health treatment providers to establish referral and care pathways and directly supporting individuals to access and engage in a wide range of support and treatment services and third sector.

**The Mental Health Nurse key duties and responsibilities will be**:

- To contribute to the quality assurance of assessments, plans and interventions to ensure they meet the required standard and to take swift remedial action when they are inadequate or of poor quality.
- To ensure that up to date, accurate case records are maintained, ensuring that information is recorded and managed within the Data Protection Act guidelines and to ensure the safety and adherence to policy guidelines of any information transferred to otheragencies.
- Reduce the number of vulnerable adults and children who are at risk of harm by identifying safeguarding concerns and making appropriate referrals.
- Identify risk and share information about vulnerable people with other agencies, to ensure risk is managed and vulnerable people are safeguarded.
- Ensure the continued promotion of the safeguarding agenda and to prioritise the well-being and safety of service users and members of staff.

**To work as an Mental Health Nurse in this unique role you will need**:

- Evidence of entitlement to work in the U.K.
- Evidence of essential qualifications
- Two satisfactory Clinical references covering the last 5 years.
- Valid NMC Pin
- The post holder will primarily be office based; however the role will require the post holder to work in other community based setting across West Yorkshire
- The post holder may be required to work flexibly across West Yorkshire on a regular basis.
