Working as part of a team within the Hospital Discharge Support (HDS) service, you will with colleagues in the community and local hospitals to support clients home from hospital.

You will co-ordinate grant programmes to provide housing support and provision, ensuring our clients have a safe and comfortable home to return to and avoid unnecessary readmission.

Working from our head office in Kemp Town, Brighton, as well as visiting local hospitals and client homes, this is an interesting and varied role. There are opportunities for flexible working, including some work from home.

**Job Types**: Full-time, Fixed term contract
Contract length: 9 months

Pay: £27,756.00 per year
