Bydd yr ymgeisydd llwyddiannus yn gweithio fel rhan o dîm er mwyn
- Cynllunio a rhoi cymorth o safon uchel a arweinir gan ganlyniadau ac sy’n canolbwyntio ar yr unigolyn i bobl sy’n defnyddio’r gwasanaeth.

Cynorthwyo a grymuso pobl sy’n defnyddio’r gwasanaethau i gyflawni eu potensial yn llawn.

Cysylltu’n effeithiol gydag ystod eang o bartneriaid mewnol ac allanol.

Sefydlu a chynnal ystod eang o gyfleoedd integredig yn y gymuned i bobl sy’n defnyddio’r gwasanaeth.

Monitro ac adolygu gwasanaethau i sicrhau eu bod yn dal i ganolbwyntio ar yr unigolyn a bod canlyniadau y cytunir arnynt yn cael eu cyflawni.

Goruchwylio gweithwyr cymorth yn effeithiol a chyfannu’r rhagweithiol at y broses Adolygu Perfformiad a Datblygu.

Dangos dealltwriaeth dda o foeseg a gwerthoedd gofal cymdeithasol gan ymrwymo iddynt.

Cyfrannu at y gwaith dyddiol o redeg y gwasanaeth dydd.
