The post of alocality mental health practitionerwithin theHeywood, Middleton and Rochdale (HMR) is an exciting opportunity to offer early help, advice and signposting to young people presenting with a variety of emotional and mental health needs at the earliest and most crucial time.

As an experienced practitioner you will be responsible for the assessment and advice of young people seeking support with their mental health needs. You will support professionals with identifying young peoples needs and ensuring they gain the right support at the right time from the right people.

Working within the wider Mental Health service offer within HMR including our early attachment team, young persons mental health support team (Schools based offer),#Thrive,CAMHS, crisis response services and our VCSE provision in the borough you will support early identification of need and support aThrivealigned service offer to ensure young peoples needs are met.
- To ensure high qualityCAMHSassessment of children and families referred to single point of access
- To provide or provide access to a range of quality psychotherapeutic interventions in accordance with individual assessed need. This will include the assessment planning implementation and evaluation of a variety of therapeutic approaches either alone or as a member of the team
- To be responsible for and manage a defined caseload and undertake case management responsibilities within the
- To identify and develop strategies to manage risk to the child and others in line with Pennine care andCAMHSprocedures
- To liaise closely with a wide range of agencies regarding individual care packages and to promote awareness of child and adolescent mental health issues within these agencies
- Undertake liaison and joint work with other professionals outside of the MDT
- To provide consultation and training on a wide range of children’s emotional health and well-being issues to professionals from a range of services including GP’s, Early Help and Primacy Care Services, Adult Mental Health Services, Youth Offending Services and Education

You will be supported to achieve success by a highly motivated local across-system leadership team who are committed to embedding a thriving culture within HMR. You will be a key member of the HMR SPOA and Locality team, receive excellent supervisory support both through clinical and management team and we will work with you to support your development. We know that the support and development of our staff is fundamental in ensuring that those that need our services get the best possible experience.
