**Job summary**:
The GPA will be part of the SDS Care Home Service. The service covers 30 care homes, both residential and nursing across all areas of Birmingham from Sutton Coldfield to Handsworth to Sheldon and Longbridge. There are approximately 1000 care home residents, each of whom are registered with West Heath surgery, who we work very closely with.

The team includes GPs, Nurse Practitioners and Admin.

The GPA will be expected to work independently, travelling to care homes covered by the service and carrying out their duties as listed, returning to West Heath surgery, which is our base, as needed.

Although part of a team, the majority of the role is working autonomously and managing a daily case load

**Main duties of the job**:
As GPAs will come from different backgrounds, different skill sets and their role as a GPA may differ significantly from a GPA in another Clinical setting, there are many factors to be considered by the Service. The GPA will support the Clinical Teams smooth running of the service by performing the more routine administration and clinical tasks on behalf of the clinicians freeing up their time to focus on the patient. Any remuneration or adaption of the job description will be at the discretion of the Service Manager and in agreement with the Human Resources and the GPA.

**Administration**:
Arranging appointments, tests and follow up appointments of patients.

Follow up on outstanding test results.

Administrative workflow tasks on the clinical system inclusive of EMIS and DOCMAN.

All forms of data entry and recording relating to patients, including scanning and summarising.

Supporting with the administration of recalls inclusive of QOF, DOAC, High risk drugs and monthly recalls.

Assisting clinicians as a chaperone where required.

Providing administrative support to clinicians and managers within the organisation.

Follow and adhere to Health and Safety procedures.

Any other delegated duties considered appropriate for the post.

Ensuring work areas are tidy including stock takes and Infection Control.

Check patient identification and confirm details with the patient and /or Care team.

Phlebotomy, urine testing, swabs, physiological measurements including blood pressure, pulse rate and rhythm, temperature, pulse oximetry, height and weight (body mass index), visual acuity in accordance with training, Policies and Procedures.

Support the Clinicians with Vaccines/wound care.

Completing QOF, DOAC High risk drugs and monthly recalls.

Prepare and maintain environments and equipment before, during and after patient interventions.

Give accurate and appropriate information to patients within own competence understanding role boundaries and limitations.

Ensure that any concerns are reported to a senior member of the clinical team in accordance with clinical pathways.

Notify a GP/Lead Nurse/ANP of any significant patient issues.

Ensure full and accurate recording in the patients computerised records.

Assist with raising awareness of health and wellbeing by participating in patient health promotion and screening initiatives in accordance with regional and national programmes and guidance.

Strict adherence to infection control in accordance with local and national policies and procedures.

**Data Protection, Confidentiality and Information Governance**:
An understanding, acceptance and adherence to the need for strict confidentiality.

Ensure all patient related information is treated sensitively and adhere to the principals of confidentiality at all times.

Work in accordance to relevant data protection legislation.

Work in accordance to Information Governance policies and procedures.

**Digital**:
Adhere to practice policies and protocols relating to I.T.

Ensuring personal passwords for computer are safe.

Adhere to practice policies relating to social media.

Checking patients details when conducting digital consultations ensuring you are speaking to the patient and not a third party.

Adhering to policies and procedures when conducting digital consultations.

**Health & Safety**:
Assist in promoting and maintaining own and others health, safety and security as defined in the practice Health & Safety Policy to include:
Responsible for the correct and safe management of the specimens process including collection, labelling, handling, use of correct and clean containers, storage and transport arrangements.

Assist in promoting and maintaining own and others health, safety and security as defined in the practice policies.

Using personal security systems within the workplace according to practice guidelines.

Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks.

Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards.
