**Role Overview**:
An exciting opportunity has arisen for a Digital and Transformation Lead to join an already established, growing and committed team in providing additional care and services across PCN patient populations. You will be responsible for leading digital transformation projects within the organisation, managing a small team of staff to assist you, and coordinating with the clinical and managerial teams to provide support within multiple projects.

This is a permanent position either full time or part time, hours can be discussed at interview. The salary is £40-48k FTE and dependent on experience. We offer 27 days annual leave rising to 33 after 10 years’ service, pro-rata.

**Key responsibilities**
- Line manage the team across PCNs and PGPA ensuring appropriate training and developments as well as operational availability.
- Ensure sharing of learning and awareness of system developments across the team.
- Improve adoption and/or optimisation of new technology to enhance patient access and experience and increase PCN productivity.
- Build relationships and facilitate collaboration between practices and the wider system to support the delivery of care to patients (including shared appointments between practices to aid delivery of enhanced access).
- Lead an evidence-based improvement approach to change including building capability for quality improvement within the PCN’s and system wide approaches to problem solving.
- Manage the whole lifecycle of a programme of work containing multiple projects, reporting on progress, and managing risks and issues, escalating where necessary to the project board, providing expertise of best practice change and project management methodologies.
- Use data, and improve data quality, to understand demand, capacity, and activity in order to hit government set targets within IIF and QOF with particular reference to Population Health Management.
- Directly or via delegation to your team, understand, interrogate, and extract data from a wide range of clinical and activity systems including but not exclusively SystmOne, EMIS, APEX Insight, Ardens, GPES, QPRS, etc.
- Directly or via delegation to your team, compile and submit reports on behalf of PCNs and PGPA to local, regional, and national governing and commissioning bodies as required either regularly or on an ad hoc basis.
- Convert raw data into business intelligence information for PCNs and PGPA
- Present information extracted and identified as a result of the teams’ efforts to PCNs and PGPA in a manner which is clearly understood.
- Lead on the identification opportunities for improvements in service through the use of existing, new and emerging technologies
- Drive improvements in patient experiences of access, and staff experience at work, to support population health management.
- Support understanding of the type and intensity of support/training needs of the PCN’s and co-ordinate this support.
- Facilitate clinically led innovation and the effective adoption of improvement initiatives, including integrated working at neighbourhood and/or place level to improve access to services for patients.
- Deliver significant projects that support the PCN’s which will lead to improvements in health outcomes for the patient population.
- Support organisational change and the uptake of initiatives that support and embed excellence within the health system.
- Review project reports and risk logs for projects, advising individual project managers on escalation process and supporting delivery of projects where there may be significant barriers to delivery.
- Responsible for the delivery of, robust, information systems to ensure that all project documentation is stored and used effectively (PID, dashboards, highlight reports, risk and issues logs etc).
- Develop programme and project management tools, report and templates that are transferable between projects and can be used for the whole project lifetime, from design to delivery.
- To ensure all public and patient contact with the office is of highest professional standard. To embed patient and public involvement at all levels of decision making.
