**What you'll be doing**

Integrated Care Across Northamptonshire (ICAN) has a focus on Community Assets Groups with the aim of supporting people with long term conditions and their carers to choose well, live well and stay well. This includes setting up and facilitating Community Asset Groups for people who are living with Dementia, Diabetes, Sickle Cell Disease, Heart Failure or other long term health conditions in the County. You will work with our partners in Health and Social Care and other organisations serving our local community to deliver informative, interactive and engaging group meetings and activities.

We continue to extend the groups that we offer throughout Northamptonshire and are influential and responsive to the strategic and local support put in place for our Carers and Cared for accessing our support at Northamptonshire Carers. We are actively playing our part to address health and social inequalities; this might be raising awareness of services needed or being solution focused and creative with our resources.

As a Group Facilitator, your role will be to:

- Create, facilitate and promote Community Asset Groups groups and activities.
- You will be expected to work with a multi-agency approach and be part of multi-disciplinary teams.
- There will also be a need to include volunteers and supporters who have lived experience.
- We will be looking to roll out new groups as well as attend events within the community.
- Creativity is key to your success in this role.
