Band 6 Quality Governance Facilitator

**Main duties include**:
- supporting all staff to actively participate in identifying, actioning and evaluating all patient safety and risk issues
- supporting all relevant staff to actively participate in the National Clinical Audit programme and make improvements based on results
- support all staff in understanding through self/peer assessment compliance with CQC standards
- managing and supporting the communication and management of the issues through Datix in line with policy
- supporting investigation leads in the investigation of serious incidents
- analysing trends to ensure actions are put in place to reduce the risk of recurrence
- writing timely reports on the Barnet Hospital site which clearly identify the issues, lessons learned and changes to practice
- supporting the triangulation all patient safety data to ensure divisional focus is appropriate to risk and protects patient safety
