As a Senior Community Care Coordinator at North East London NHS Foundation Trust, you will be responsible for leading and managing a Community Nursing Team, providing high-quality nursing care to patients and their families in the community.

This is a fantastic opportunity for a motivated and experienced Registered Nurse to work in a dynamic environment, utilizing your skills and expertise to make a real difference in the lives of our patients.

You will be responsible for managing a caseload, working closely with GPs, other professionals, and statutory agencies, and ensuring the provision of a high standard of nursing care. We are looking for someone with excellent leadership skills, the ability to work independently, and a strong understanding of clinical governance priorities.

The ideal candidate will have a good range of clinical skills, including assessment, and experience of team working, interagency collaboration, and teaching/assessing individuals. Knowledge of the NHS Plan, NSF, and clinical governance priorities is also essential.

This is a Band 6 position, and the estimated salary for this role is £38-£44 per hour.
