**Job Summary**

To be responsible for the provision of paramedical services to Copeland population residing within the community. Following GP triage the post holder will be expected to undertake assessment, diagnosis and consider treatment options or referral of patients utilising support from the GP where necessary for those presenting acutely with on the day needs.

The post holder will demonstrate a caring, compassionate attitude as an experienced practitioner who, acting within their professional boundaries, will provide care for the presenting patient. This will be predominantly face to face or possibly via the telephone. They will demonstrate safe, competent clinical decision making and expert care. The post holder will communicate and work collaboratively with the practice teams.

The role will be mobile and flexible; the paramedic will operate on behalf of all practices within Copeland, assessing and treating patients in their own home or care home.

The role forms part of the developing on the day primary care service in Copeland, and the post holder will play a part of shaping the service and the role.

You will be expected to work flexibly as part of a team covering hours of work between 9am and 6.30pm Monday to Friday, initially however these hours may be extended to 8pm.

**KEY RESPONSIBILITIES**

**Clinical**
- Work as part of a multi-disciplinary team to deliver safe and high quality care
- To accurately triage and prioritise patients, including in emergency situations, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical and drug history.
- Recognise, assess and refer patients presenting with mental health needs
- Diagnose and manage acute and chronic conditions, including drug and non-drug treatment methods into a management plan within scope of practice.
- To evaluate clinical information from examination and history taking, and initiate appropriate treatment and/or referral
- To undertake and interpret diagnostics and take appropriate action to manage patient care, whilst distinguishing between abnormal and normal findings in order to relate them to a possible diagnosis.
- To signpost and refer appropriately to other services in the identified healthcare pathway, including active liaison with agencies where required (e.g. vulnerable patients)
- Support patients in the use of their prescribed or over the counter medications
- Chaperone patients where necessary
- Adhere to patient group directives and local clinical pathways
- To maintain accurate and systematic documentation of patient interactions (including using appropriate read codes) and clinical care provided, including adverse events.
- To advise and support members of the public on healthy lifestyles, health promotion and disease prevention.
- To provide clinical advice to other members of the team if appropriate.

**Communication**
- To liaise and interact with multi-professional teams across organisational boundaries, including primary care, social services and secondary care providers according to patient needs and appropriate care pathways.
- To maintain communication with all healthcare professionals and outside agencies to ensure seamless, patient-centred service provision.
- To ensure effective communication so that the patient receives the appropriate level of care and wherever possible avoid unnecessary hospital admission.
- To take an active role in maintaining expert communication with all patients, carers and relatives which meets their individual needs.
- Communicate confidential and sensitive information to patients, relatives and carers in relation to their condition
- To deal with complaints in a calm and courteous manner, ensuring that, wherever possible, complaints are dealt with efficiently and satisfactorily on a local level.
- To communicate effectively in demanding and challenging situations, where there may also be hostility
- To maintain contemporaneous accurate and systematic documentation of patient consultations, consistent with legislation, policies and procedures.

**Professional**
- To work collaboratively with other members of the integrated care community team care team such as the District Nursing, Mental Health Services, Care Homes, Acute hospital teams etc.
- To ensure that the wellbeing of patients and staff is maintained in accordance with policies.
- To participate in clinical audit in line with the PCN clinical audit plan.
- To contribute to local and national practice targets e.g. QOF.
- To monitor and meet care outcomes against standards and recommend change as necessary
