We are looking for an enthusiastic individual to join our team as a Band 3 Patient Flow Coordinator in our Emergency Department.

You must be a self-motivated individual with proven communication skills and the ability to work with initiative. You will be calm under pressure as this is a very busy and at times demanding environment with a willingness to undertake and learn new tasks.

You must also have strong inter-personal skills and strong organisational skills will be essential as you will be dealing with doctors, other health care professionals, and acutely unwell patients. Broad experience of Trust IT systems and experience in an Acute setting is also desirable.

You will support the Emergency Department to ensure accurate, timely data input and flow support, in line with the trust values, to prevent unnecessary delays and improve patient experience. This is a patient facing role and you will spend the majority of your time liaising with patients, families and with members of your team.

The patient flow coordinator will escalate potential delays in a timely manner working closely with doctors, nurses and support staff. In addition, you will help to prevent delays as much as possible by following up outstanding requests to the relevant individuals.

- Ability to work as part of a team liaising with inter-disciplinary team members regarding flow support and timely and accurate data input.
- Ensure completion and delivery of relevant documentation to assist with the discharge process/ flow support.
- Communication with patients, relatives and other staff members concerning issues related to patients stay. Ability to deal with communication issues.
- Liaise with other agencies/patient/relatives in order to provide effective communications to assist with timely discharge of patients.
- Completion of relevant patient documentation in order for an effective discharge process/ onward flow out of ED.
- Ensure that the social circumstances / discharge arrangements of each patient have been assessed and documented within 24 hours of patient being admitted to the ward environment.
- Monitor the progress of referrals/assessments that are required to assist with the discharge process, either to the wards to or home. For example, scans, specialist reviews, test results etc.
- Follow up any outstanding requests that are causing a delay.
- Support ED team by facilitating the process of requesting TTO’s / Transport/ Discharge Summary to assist with a timely discharge and avoidance of any delays.
- Ensure that any grab packs and paper documents relating to the admission and discharge process are replenished for use by ED team.
- Ensure that all relevant paperwork is completed by the appropriate persons to ensure an efficient and timely discharge.
- Support chasing results for tests where this could delay achieving discharge.
- May, on occasion be required to prepare bed spaces in order for the admission of patients to support the ED team
- May on occasion be required to support transfer of patient to other ED area, ward or discharge lounge
- Operating telephone and computers.
- Walking through the department at intervals updating patient locations and to obtain accurate capacity picture for new admissions
- Maintain accurate record keeping regarding the discharge process for each patient.
- Provide all patients with a leaflet explaining the discharge process as per hospital policy.
- Liaise directly with patients and their carers regarding the discharge process.
- To ensure that relevant policy and pathways are adhered to at all times.
- Support delivery of organisational objectives, achieving quality outcomes and patient centred care
- Highlight any issues to the appropriate Senior Manager that may affect the discharge process, for example a lack of senior doctors, therapy services and delays in test results/referrals.
