The post holder will work autonomously within the Complex Care at Home Multi -Disciplinary Team to deliver bespoke dietetic interventions to patients with multiple comorbidities - These clients are often frail and may have cognitive or memory problems. The post holder with have an advanced level of professional knowledge to lead in the development and delivery of expert nutrition and dietetic support to patients, carers and health care teams. They will provide advice, consultation and training clinical supervision to other members of the team for their provision of nutritional and dietetic based interventions to help improve patients physical and psychological health. They will provide advice, consultation and training to staff working with patients across a range of agencies and settings, where appropriate.
