An exciting opportunity has arisen to become part of the Band 7 team within the Adult Critical Care Outreach Team at Nottingham University Hospitals NHS Trust (NUH). As one of the largest Trusts in the UK the Critical Care Outreach Team provides a diverse and comprehensive CCOT service to all adult patients within NUH which includes specialities such as; Neurosurgery, Haematology, Major Trauma, Renal and Infectious diseases.

The Critical Care Outreach Team (CCOT) at NUH is a highly regarded team who provide a Trust wide resource covering all adult patient areas across our City and Queens Medical Centre campuses, in total providing access to the Critical Care Outreach service to more than 80 wards and clinical areas. The team is multi-professional consisting of highly experienced and skilled Band 7 and 6 nurses, specialised physiotherapists and a CCOT assistants.

Main duties of the job

As a Band 7 you will lead the clinical service, providing ward based advanced clinical assessments, patient interventions and develop management plans with the goal to halt or reverse clinical deterioration. You will also have an opportunity to help shape and develop the service taking on managerial and leadership responsibilities to ensure that the service evolves to meet our patients and service user's needs.

The post holder will be required to demonstrate advanced clinical assessment skills and develop extensive knowledge of common clinical situations encountered within the range of specialities across the trust in particular; management of hypoxia with nasal high flow oxygen therapy, non-invasive ventilation, tracheostomy care and weaning, sepsis management and transfer of the critically unwell patient.

In addition you must have the ability to prioritise the workload of the team as the shift co-ordinator, provide both formal and informal teaching to staff at all levels, resource management such as rota co-coordination, sickness management and contribute to the ongoing development of the service and team members using staff development plans and appraisals.

Working for our organisation

The Critical Care Outreach service provides an extensive 24 hour, 7 day service, 365 days a year, across both City and Queens’s campuses. The post holders will therefore be expected to work a combination of long day and night shifts.

All staff work across both campuses in order to provide a truly flexible service to meets the ever changing needs of the critically ill/ deteriorating ward patient throughout NUH.

Applicants will need to have previous Critical Care Outreach experience and have undertaken a degree and critical care course (or equivalent).

You will be supported with a formal induction and training programme, complete Critical Care Outreach clinical competencies, work alongside the teams Clinical Nurse Educator and access a Critical Care Outreach specific advanced training programme.

Detailed job description and main responsibilities

KEY JOB RESPONSIBILITIES

**Clinical**

Demonstrate the use of evidence-based specialist skills and practice in the care of critically ill ward-based patients. Participate in key developments relating to CCOT so that high quality patient focused services are promoted.

Demonstrate professional autonomy and the ability to lone work at a higher level, exhibiting effective decision making skills.

Use of specialist knowledge and skills in the assessment, planning, implementation and evaluation of care delivered to the critically ill/deteriorating ward based patient, ensuring the appropriate documentation is maintained and completed.

Demonstrate the interpretation and delivery of appropriate action / treatment / therapy to the critically ill/deteriorating adult patient, ensuring records are updated and maintained.

Monitor and evaluate the condition of patients using the national early warning scoring system, recognising and alerting staff to changes in the patients’ condition.

Assist and participate in the follow up care of patients discharged from critical care.

Assist and participate in the transfer of the critically ill/deteriorating ward patient.
