**SUMMARY PURPOSE OF ROLE**

The Community Worker provides a free preventative welfare support and advice service on a confidential one-to-one basis to any person who is using services provided by Teesdale Day Clubs, to reduce feelings of isolation and loneliness, and promote independent living, positive wellbeing and maintenance of good health.

The Community Worker will also work closely with Club Leaders to ensure that any lunch club member who requires initial help or encouragement to attend a lunch club, or needs contact during any temporary absence (e.g., whilst in hospital), are supported. They will work with Club Leaders to address any barriers that people are experiencing with regard to accessing our services (e.g., financially affording the lunch club weekly charge).

The Community Worker will also provide anyone using our services with information that is supportive and assists them to make their own decisions about their life, and will signpost/refer people to other community services or specialist support as required.

**KEY OBJECTIVES**

Provide a one-to-one service by visiting, contacting, signposting, accompanying, or supporting members on any day-to-day issues that are affecting their wellbeing, general health and happiness.

To provide a confidential liaison point with relatives or carers when appropriate.

To support the work of Club Leaders by providing information and assisting as appropriate.

Work closely with Club Leaders to ensure early identification of member needs, barriers, or personal issues that may affect their safety (including safeguarding), health, well-being, or ability to participate in activities we offer.

Carry out effective networking and public relations activities as appropriate that ensure the maintenance of good partner relations and partnership working, attracting referrals by partner agencies or facilitating self-referrals to our services.

**PRINCIPAL DUTIES AND ACCOUNTABILITIES**

**Members support service**

Provide a one-to-one home visiting and telephone community support service that may include the following (not an exhaustive list):

- Wellbeing support through active listening
- Signposting to external agencies or specialist support
- Liaising with family members or carers to raise or mitigate concerns
- Accompanying to critical health or other appointments
- Providing informal advocacy or liaising with external agencies on behalf of a member (where express permission is given)
- Taking any reasonable and appropriate action that is in the best interest of the individual that enables them to maintain their chosen lifestyle and level of activity, and cope with day-to-day life
- Making hospital or care home visits as appropriate to existing members

Getting to know members well so that their needs and interests are considered when support is offered

Visit each club on a rotational basis to ensure that everyone has equal access to support available and so that relationships are developed with people to break down barriers and create a safe, familiar, and approachable support service

Keep accurate case records (data) for all supported persons, including maintaining community worker activity outcomes data, performance measurement information, and any other data required by any funder or funding monitoring requirements, as directed by the manager or Trustees

Support all club personnel to be effective in the support of members, working with the manager and Club Operations Coordinator on any issues that arise in relation to people’s safety and happiness

Assisting Club Leaders as appropriate by accompanying people on arranged trips or alternative activities, to ensure that any at risk or frail people are safe and have an enjoyable and beneficial experience

Provide a central resource to Club Leaders to ensure that information and activities available to lunch club members are circulated and visible to all

To carry out any other tasks commensurate with this role as requested by the manager or trustees to ensure the smooth operation of the charity

**ESSENTIAL**

A warm and friendly demeanour and the ability to self-organise is essential to this role

Knowledge of the importance of safeguarding, confidentiality, and good record keeping that complies with GDPR and best professional practice
