We are looking to appoint a Vascular Access Pathway Navigator to support the vascular access pathway for Renal & Transplant patients in the West London Renal Transplant Centre (WLRTC). The post holder will primarily help provide a seamless service for patients referred for vascular access appointments and theatre dates.

You will be able to work both autonomously using your own initiative and within the team in the development and delivery of the vascular access service. This role entails working collaboratively within the renal department’s administration team as well as providing the support to the vascular access clinical team.

You will be responsible for managing the day to day organisation and smooth function of the vascular access pathway utilising designated operational practices within the bookings administrative team. Therefore, it is expected that you will be computer literate, an excellent communicator with the ability to build relationships across staff groups. You will be a highly organized individual working with attention to detail and able to manage competing priorities and use your own initiative to solve problems.
- Act as the single point of contact to coordinate referrals for Vascular Access service and MDT.
- Liaise with the referrers, clinicians and patients to ensure that vascular access appointments and theatre slots are used effectively and highlight the need for extra clinics/lists to cover changes in demand and capacity.
- Assist in co-ordinating patient bookings, activity and special requests within the vascular access service.
- Assist in the preparation for patients to be discussed in the weekly vascular access MDT and tracking of patient’s progress within the diagnostic pathway.
- Actively engage in the wider booking and re-booking process.
- Update and maintain all databases in a timely fashion ensuring accuracy of data.
- To assist in audit and data collection relating to imaging services
- Assist with the monitoring of waiting times targets and escalate risks relating to patient pathways and waiting times the Business Manager and the Vascular Access Leads.
- To utilise finite capacity within the Vascular Access service, bringing patients appointments forward where necessary, flagging up capacity issues and performance target issues.
