We have an exciting opportunity for a Patient Care Navigator on our short stay unit. We are a 32 bedded unit caring for patients from medicine, surgery, ENT and orthopedics. Due to the fast paced nature of the ward we are looking for a proactive patient care navigator to assist with safe, efficient discharges and support with flow throughout the hospital.

1. Provide administrative support to the department to support the smooth flow of patients through
the ward.
2. Attend ward rounds to support the medical and nursing teams in effective patient flow
3. Assist/Lead in facilitating board rounds supported by medical, nursing, and wider multidisciplinary team.
4. Liaise with other departments to ensure timely investigations are in place and available.
5. Escalate barriers to timely diagnostics, assessments and reviews to the appropriate team.
6. Prompt medical teams re use of EDD, ensuring it is captured within the Web V system and reviewed
at each ward / board round.
7. Review all patient’s at afternoon safety huddle, escalating any barriers highlighted e.g. social,
procedures, delays to discharge
8. Review patient journey daily against documented EDD and discuss with the Medical Team
accordingly.
9. Responsible for keeping the Web V electronic system up-to-date at all times, SNCT and Red2Green.
10. Deal with general queries from patients, relatives and other departments.
11. Review diagnostic test requests (e.g. blood tests, x-rays) ensuring results are brought to the
attention of the relevant Medical or Nursing staff in a timely manner and are acted on.
