To support a quality service across the Trust, for patients following hip fracture by promoting an integrated pathway that is understood by all involved and meets the complex needs of these patients.

To be responsible for continual inputting and updating the requirements of the National Hip Fracture Database

The post holder will also be responsible for ensuring accurate and timely inputting of patient data into the relevant clinical systems for the capturing of patient details and outcomes, adhering to trust data quality, health records and information governance policies

Reporting to managers any identified gaps in the agreed pathway and database which will ensure patients receive quality care with maximum compliance across the pathway.

To work as part of the multidisciplinary team, acting as liaison point between Orthopaedic Consultants, theatre staff, ward staff, A&E, Orthogeriatricians, therapists and trauma coordinator

To support the MDT coordinator as required in managing the Fragility hip fracture pathways

It is expected that the post-holder will manage their workload in an effective and efficient way and have a flexible approach to covering the role, as it continues to develop with the ever-changing needs of the business.

To be responsible for managing own time management and prioritising workday.

1. A key responsibility for the post holder is to ensure the timely and accurate inputting of data on the patient information management
2. Audit the progress of the hip fracture patients and complete the NFHD tool with attention to detail and accuracy and produce reports as requested and present to Fragility Hip Fracture Monitoring group.
3. To liaise with the data quality department to report any anomalies’ found in any patient data on the trusts clinical systems, highlighting errors or concerns.
4. Regularly attend and contribute to the hip fracture monitoring group and the MDT mortality review group.
5. To ensure that at all times there are completed blank integrated patient pathway folders available for use
6. Monitor and report to senior ward staff compliance to completion of the integrated pathway in relation NFHD requirements.
7. Support the ward clerk with the filing and updating of the integrated patient pathway
8. Regularly monitor #NOF patients across the hospital and ensure that the MDT are aware and that the outlier ward fully understand and utilise the integrated pathway.
9. Ensure that all patients are in receipt of the hip fracture leaflet and the MDT are aware of patients/families requiring further information
