**Purpose of post**

The post holder will provide information, advice and support to young people (and their families) who have a diagnosis of pre-diabetes or Type 2 Diabetes. The post holder will also be providing information to young people, families and their support networks to decelerate the numbers of young people with obesity and at risk of becoming diabetic. The advice and support will enable changes in behaviour and lifestyle to produce successful outcomes.

**What we are looking for**

You will have a passion for helping young people with diabetes manage their condition and lead healthy lives. You should have a strong understanding of diabetes and its effects on the body, as well as experience working with young people. Excellent communication skills are also essential, as you will need to build relationships with young people, their families, and healthcare professionals to deliver effective support and education. A proactive and creative approach to problem-solving, as well as the ability to work independently and as part of a team, are also important qualities for this role.

**Main Objectives**
- Continue our co-production exercise with children, families and carers to understand their needs around managing their condition to inform delivery, education and levels of support.
- Use this information to plan and deliver education on Type 2 Diabetes and pre-diabetes, healthy eating, weight management for our target communities using the existing network of support services including Young People Services, Family Hubs, Health Visitors, the Children and Family Wellbeing Service, School Nurses and VCFSE organisations.
- Deliver a targeted programme to families with children and young people with Type 2 Diabetes and pre-diabetes, aimed at those with level 2 and level 3, offering practitioner lead support using a mix of community health champion support (level 2) and more in-depth 1-1 support for level 3 young people.
- In conjunction with health partners - develop a Toolkit for staff, volunteers, and community health champions to provide young people, families and their wider support network with relevant, timely and up to date information on Type 2 Diabetes and weight management in order to (1) address identified health inequalities for children and young people with Type 2 diabetes and (2) prevent more young people from becoming diabetic.
- Develop a forum of Youth Community Health Champions to provide peer support and CYP expertise to support this (and other health related) agenda.
- Utilise our existing network of trained volunteers and community health champions (who are experienced in working with families) to deliver key health messages and advice on condition management and the other social determinants that affect health, change behaviour and long-term habits to improve longer term outcomes and impact.

**Duties and responsibilities**
- Use knowledge and experience to make sound decisions and to adapt practice to meet individual circumstances.
- Promote and support self-management and engage with patient support groups.
- Engage and hold events for awareness of diabetes for the young people and families across Blackburn with Darwen and East Lancashire.
- Provide support and direction to patients/carers and staff with regard to the management of patients with diabetes across Pennine Lancashire.
- Participate in patient group education.
- Promote better understanding of diabetes for patients and their families and other agencies involved with the patient through education, support and counselling.
- Liaise with and formally refer clients to the multidisciplinary and paediatric team.
- Understand the NICE guidance and the evidence to support clinical practice.
- Attend regular team meetings and other meetings as required.
- Work in accordance with guidelines and protocols to deliver a standardised approach to patients including care planning, signposting and education.
- Able to manage clinical and non-clinical priorities on a day to day basis
- Participate fully in the IMO’s clinical supervision and appraisal schemes.
- Takes responsibility for personal development and education including continuing education and development of a professional profile.
- Tackle discrimination and harassment, and promote equality and diversity in the workplace.

**Education / Qualifications**

**Essential**
- NNDP trained and accredited.
- Evidence of continuing professional development Diabetes Care.
- Good level of secondary education Minimum of 7 GCSE’s or above or equivalent.

**Desirable**
- Evidence of audit and data analysis.
- Post graduate certificate in Education
