We are advertising for Nurses and AHPs who are experienced in stroke care who wish to pursue a career as a specialist in stroke by completing a Maters Degree Programme as an Advanced Care Practitioner.

This is a fully funded course and you will have 20% "off the job" learning as well as being fully supported with your studies.

The minimum contract for this training post is 30 hours per week.

You will be working 11.5hrs a shift and working a shift pattern that covers seven days a week, 24 hours a day. This includes weekends and bank holidays.

You will participate in assessing patients in the Emergency Department, requesting scans, delivering thrombolysis, supporting the flow of patients through the stroke service and participating in the thrombectomy procedure.

You will also be supporting more junior staff in the care of acute stroke patients including post ITU patients, post hemi-craniectomies, naso-gastric tubes and tracheostomies.

This is a busy and dynamic role where two days are never the same. We currently have seven trainee ACPs who all thrive in the role and are clinically expert at what they do. They are all here to support you as well as our stroke Consultants.

Clinical

ThetACPpost holder will be supported to develop skills, practices and behaviours which
- Practice in compliance with their respective code of professional conduct, being responsible and accountable for their decisions, actions, or omissions at this level of advanced practice
- Work towards demonstrating a critical understanding of their broadened level of responsibility and autonomy, recognising the limits of their own competence and scope of practice, to include when working with complexity, risk or uncertainty
- Develops own professional judgment skills identifying when to escalate and seek help/guidance, critically reflecting on practice, self-awareness, emotional intelligence and their willingness to change or challenge thought processes
- Learn to work in partnership with individuals, families and carers, using a range of assessment methods as appropriate (e.g. history-taking and physical assessments, holistic assessments, identifying risk factors, mental health assessments, requesting/undertaking and/or interpreting diagnostic tests, and conducting health needs assessments).
- Develops effective communication skills, supporting people in making decisions, planning care or seeking to make positive health changes, promoting person-centred approaches in health and care
- Develops effective communication skills with colleagues both internally and/or externally, in verbal, non-verbal and written communication methods, adapting to changing parameters as required; this includes thorough accurate documentation in patient records as required
- Provides and receives highly sensitive, complex or contentious information relating to patient care, and communicates information to patients, families or carers with respect, compassion and empathy
- Develops proficiency and clinical acumen in relation to decision-making skills, to inform clinical reasoning approaches when dealing with differentiated and undifferentiated presentations and complex situations; synthesising information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses, knowing when to refer on to a speciality or escalate care
- Works towards increasing knowledge and skills in advanced clinical practice, in order to initiate, evaluate and modify a range of interventions, which may include the prescribing of medications, therapies, lifestyle advice and care
- For registered professionals that are able to prescribe, in roles requiring therapeutic medications and treatments, and after approval from the Trust’s Non-Medical Prescribing Lead, the trainee will undertake the Non-Medical Prescribing Module (NMP V300) as part of the ACP MSc pathway, where after the trainee will record the achievement on their regulatory body’s professional registration
- Develops professional judgement to manage risk appropriately with support and clinical supervision, especially where there may be complex and unpredictable events, promoting the safety and wellbeing of individuals, families and carers
